This Health Data Policy does not apply to information or practices that are not subject to laws that govern Health Data, such as publicly available information, job applicant and employee information, information collected within certain clinical or peer-reviewed research settings, or information that is otherwise exempt. A. The Categories of Health Data We Collect
While the data we collect depends on the context of your interactions with us, we may collect the following categories of Health Data:
· Individual health conditions, treatment, diseases, or diagnosis;
· Bodily functions, vital signs, symptoms, or measurements of the information described in this section, such as body weight, height, diet, blood tests, urine tests, and saliva tests;
· Genetic data;
· Data that identifies a consumer seeking healthcare services; and
· Information processed to associate or identify a consumer with the data described above that is derived or extrapolated from non-health information.
B. Purposes of Collection, Use, and Disclosure of Health Data
Generally, we may collect, use, and disclose the Health Data that we collect for the purposes set forth below:
· As reasonably necessary to provide you with the product and Services you have requested;
· To provide support in connection with the use of our products and Services;
· To communicate with you;
· To establish and manage your account;
· To better understand how users access and use our Services, products, and offerings, and for other research and development activities and similar analytical purposes, such as to evaluate and improve the Platform, our Services and business operations, including to develop and enhance our Services (which involves working with analytics vendors that may collect Health Data about you over time and across different Internet websites or online services including this Site and the Services), as well as analyzing and development features and new products and offerings, and for internal quality control and training purposes;
· For research and product development, including to train or build our artificial intelligence/machine learning (“AI/ML”) algorithms and tools, and for other AI/ML development activities.
· To tailor the content that we may send you, including to offer personalized help and instructions, and to otherwise personalize your experience;
· For marketing and advertising, and for other promotional purposes;
· For market research and surveys;
· For insight development and data enhancement purposes;
· For planning and managing events;
· To protect our Services and our business operations; to prevent and detect fraud, unauthorized activities and access, and other misuse, where we believe necessary to investigate, prevent or take action regarding illegal activities, suspected fraud, situations involving potential threats to the safety or legal rights of any person or third party, violations of our Terms of Use or other agreements we have with you, to defend our rights, including through legal or other dispute resolution processes.
· To comply with the law and our legal obligations, to respond to legal process and related legal proceedings, and government or law enforcement requests;

In general, we may collect or receive Health Data from the following categories of sources:
· Directly from you (such as when you complete certain forms or surveys) or indirectly from you (such as through your devices);
· Service providers who perform functions on our behalf, like IT and website hosting and data storage, and other parties that support the Services, such as organizations that provide genetic testing laboratory services;
· Those authorized to provide Health Data on your behalf; and
· Marketing and analytics providers.

We may share the following categories of Health Data with third parties and affiliates:

E. The Categories of Third Parties and Specific Affiliates with Whom We Share Health Data
We may share the categories of Health Data included in Section D with the following categories of third parties and specific affiliates:
· Data analytics providers;
· Government entities and regulators;
· Advisors, agents, and consultants;
· Operating systems and platforms;
· Third parties that support certain testing or analysis functions of the Services, such as genetic testing or laboratory services
· As legally required in relevant legal proceedings and otherwise to the extent required or explicitly permitted by applicable law, including cooperating with law enforcement requests and investigations where we believe doing so is necessary to protect the Services, our rights and property, or the rights, property, and safety of others;
· Others as required by law; and
· Third parties we, our affiliates, or subsidiaries are or may be acquired by, merged with, financed by, or invested in by another company, or if any of our assets are or may be transferred to another company, whether as part of a bankruptcy or insolvency proceeding or otherwise, we may transfer the information we have collected about you to the other company. We may also share certain information as necessary prior to the completion of such a transaction or corporate transactions such as financings or restructurings, to lenders, auditors, and other advisors, including attorneys and consultants, as part of due diligence or as necessary to plan for a transaction.
F. Your Privacy Rights
Depending on your place of residency, you may have some or all of the below rights regarding your Health Data, subject to applicable law and certain exceptions. We reserve the right to respond to your request in accordance with applicable law.
Exercising Requests. To make a request to exercise any right with respect to Health Data about you, please email us at support@voloridgehealth.com. Before completing your request, we may need to verify your identity or your authority to make a request on behalf of another person. We may send you a link to verify your email address and may request additional documentation or information solely for the purpose of verifying your identity, depending on the type of your request.
WASHINGTON RESIDENTS
● Right to Access and Confirm Sales and Other Activities: If you ask us, we will confirm whether we are collecting, sharing, or selling your Health Data. You may request that we provide you with a copy of the Health Data that we maintain about you. At your request, we will provide a list of all third parties and affiliates with whom we have shared or sold your Health Data.
● Right to Withdraw Consent: Where we have relied on your consent or authorization for certain processing of your Health Data, you may withdraw that consent or authorization at any time, including if we requested your authorization to “sell” your Health Data.
● Right to Delete: You may request that we delete the Health Data we maintain about you.
● Right to Appeal: If we deny your request to exercise any of the above rights, you may request to appeal this decision.
If your request to exercise a right with respect to your Health Data is denied, you may request to appeal this decision with us. If your appeal is unsuccessful, you can file a complaint with the Washington Attorney General’s Office by clicking here.
NEVADA RESIDENTS
● Right to Access and Confirm Sales and Other Activities: If you ask us, we will confirm whether we are collecting, sharing, or selling your Health Data. At your request, we will provide a list of all third parties with whom we have shared or sold your Health Data.
● Right to Correct: If your Health Data that we maintain is inaccurate and you would like us to correct it, you may request that we make changes to it.
● Right to Withdraw Consent or Stop Other Processing: Where we have relied on your consent or authorization for certain processing of your Health Data, you may withdraw that consent or authorization at any time. You may request that we not sell any Health Data about you that we have collected now or will collect about you. You may also request that we stop collecting or sharing any Health Data about you.
● Right to Delete: You may request that we delete the Health Data we maintain about you.
● Right to Appeal: If we deny your request to exercise any of the above rights, you may request to appeal this decision.

NEW YORK RESIDENTS
● Right to Access: You may request that we provide you with a copy of the Health Data that we maintain about you.
● Right to Withdraw Consent: Where we have relied on your consent or authorization for certain processing of your Health Data, you may withdraw that consent or authorization at any time, including if we requested your authorization to “sell” your Health Data.
● Right to Delete: You may request that we delete the Health Data we maintain about you.
● Right to Appeal: If we deny your request to exercise any of the above rights, you may request to appeal this decision.

This Health Data Policy is current as of the effective date set forth above. We may change this Health Data Policy from time to time, so please be sure to check back periodically. We will post any updates to this Health Data Policy on this page. If we make material changes to how we collect, use, or disclose the Health Data that we have previously collected about you, we will endeavor to provide you prior notice, such as by emailing you or posting prominent notice on our website or within the Services.
